Nabiximols (brand name Sativex) is a patented medicine containing CBD and THC in equal proportions. The drug was approved by Health Canada in 2005 for prescription to treat central neuropathic pain in multiple sclerosis, and in 2007 for cancer related pain. In New Zealand Sativex® is approved for use as an add-on treatment for symptom improvement in patients with moderate to severe spasticity due to Multiple Sclerosis who have not responded adequately to other anti-spasticity medication and who demonstrate clinically significant improvement in spasticity related symptoms during an initial trial of therapy.
In Canada, the methodology used for analyses and sample collection for THC analysis of hemp plantings is standardized (at the Health Canada/Therapeutics Program/Hemp web site at www.hc-sc.gc.ca/hpb-dgps/therapeut/htmleng/hemp.html, see “Industrial Hemp Technical Manual” for procedures on sampling plant materials and chemical procedures for determining THC levels). The regulations require that one of the dozen independent laboratories licensed for the purpose conduct the analyses and report the results to Health Canada. Sample collection is also normally carried out by an independent authorized firm. The Canadian system of monitoring THC content has rigidly limited hemp cultivation to cultivars that consistently develop THC levels below 0.3%.
Yet, even with this progress, hemp businesses seem to face difficulty expanding in the US as they face challenges in traditional marketing and sales approaches. According to a case study done by Forbes, hemp businesses and startups have had difficulty marketing and selling non-psychoactive hemp products, as some online advertising platforms and financial institutions do not distinguish between hemp and marijuana.
Last year, the National Academies of Sciences, Engineering and Medicine released a nearly 500-page report on the health effects of cannabis and cannabinoids. A committee of 16 experts from a variety of scientific and medical fields analyzed the available evidence — more than 10,000 scientific abstracts in all. Because so few studies examine the effects of CBD on its own, the panel did not issue any findings about CBD specifically, but it did reach some conclusions about cannabis and cannabinoids more generally. The researchers determined that there is “conclusive or substantial evidence” supporting the use of cannabis or cannabinoids for chronic pain in adults, multiple sclerosis-related spasticity (a kind of stiffness and muscle spasms), and chemotherapy-induced nausea and vomiting. The committee also found “moderate” evidence that cannabis or cannabinoids can reduce sleep disturbances in people with obstructive sleep apnea, fibromyalgia, chronic pain and multiple sclerosis, as well as “limited” evidence that these substances can improve symptoms of Tourette’s syndrome, increase appetite and stem weight loss in people with HIV/AIDs, and improve symptoms of PTSD and anxiety.
Until very recently, the most convincing evidence that cannabis use precipitates schizophrenia came from a 15-year prospective study of cannabis use and schizophrenia in 50 465 Swedish conscripts (Andreasson et al., 1987). This study investigated the relationship between self-reported cannabis use at age 18 and the risk of being diagnosed with schizophrenia in the Swedish psychiatric case register during the next 15 years. Andreasson and colleagues found a dose–response relationship between the risk of schizophrenia and the number of times cannabis had been used by age 18 (1.3 times higher for those who had used cannabis 1–10 times, 3 times higher for those who had used cannabis 1–50 times, and 6 times higher for those who had used cannabis more than 50 times). These risks were reduced after statistical adjustment for potentially confounding variables (a psychiatric diagnosis at age 18, and parental divorce), but the relationships remained statistically significant.

The focus of public health interventions is to prevent and manage diseases, injuries and other health conditions through surveillance of cases and the promotion of healthy behavior, communities, and (in aspects relevant to human health) environments. Its aim is to prevent health problems from happening or re-occurring by implementing educational programs, developing policies, administering services and conducting research. In many cases, treating a disease or controlling a pathogen can be vital to preventing it in others, such as during an outbreak. Vaccination programs and distribution of condoms to prevent the spread of communicable diseases are examples of common preventive public health measures, as are educational campaigns to promote vaccination and the use of condoms (including overcoming resistance to such).
Cannabis has mental and physical effects, such as creating a "high" or "stoned" feeling, a general change in perception, heightened mood, and an increase in appetite. Onset of effects is within minutes when smoked, and about 30 to 60 minutes when cooked and eaten. They last for between two and six hours. Short-term side effects may include a decrease in short-term memory, dry mouth, impaired motor skills, red eyes, and feelings of paranoia or anxiety. Long-term side effects may include addiction, decreased mental ability in those who started as teenagers, and behavioral problems in children whose mothers used cannabis during pregnancy. There is a strong relation between cannabis use and the risk of psychosis, though the cause-and-effect is debated.

Safety studies of cannabidiol showed it is well-tolerated, but may cause tiredness, diarrhea, or changes in appetite as common adverse effects. Epidiolex documentation lists sleepiness, insomnia and poor quality sleep, decreased appetite, diarrhea, and fatigue.

However, it doesn’t make sense nor is it fair to put all the blame to cannabis. There is some speculation that George Washington smoked the flower of the cannabis plant in order to achieve a recreational high ("Like all farmers, Washington probably sampled the quality and potency of what he grew, and he may have used this hemp to treat his chronic tooth aches"), but there is no evidence in any of his writings that he grew hemp for anything other than industrial purposes. It is sometimes supposed that an excerpt from Washington's diary, which reads "Began to seperate [sic] the Male from the Female hemp at Do.&—rather too late" is evidence that he was trying to grow female plants for the THC found in the flowers. However, the editorial remark accompanying the diary states that "This may arise from their [the male] being coarser, and the stalks larger" In subsequent days, he describes soaking the hemp (to make the fibers usable) and harvesting the seeds, suggesting that he was growing hemp for industrial purposes, not recreational.
Buying online is less reliable still because there’s no regulation or standardization. What you see on the label may not be what you are getting. A 2017 study in JAMA found that of the 84 CBD products researchers bought online, 43% had more CBD than indicated, while 26% had less, and some had unexpected THC. “There’s a 75% chance of getting a product where the CBD is mislabeled,” says Marcu, one of the study’s coauthors.
The Senate is also calling for marijuana tax revenue to be allocated to “fund public education, job training, reentry services, drug treatment and prevention programs, community-based supportive services, improvements to the Metropolitan Transportation Authority (MTA) and to expand training for state and local law enforcement to maintain road safety.”
Market considerations also heavily determine the wisdom of investing in hemp. Growing hemp unfortunately has a magnetic attraction to many, so there is danger of overproduction. A marketing board could be useful to prevent unrestrained competition and price fluctuations, but is difficult to establish when the industry is still very small. As noted above, unwise investment in Canada produced a glut of seeds that resulted in price dumping and unprofitable levels for the majority. Cultural and production costs of hemp have been said to be comparable to those for corn, and while the truth of this remains to be confirmed, the legislative burden that accompanies hemp puts the crop at a unique disadvantage. Among the problems that Canadian farmers have faced are the challenge of government licensing (some delays, and a large learning curve), very expensive and sometime poor seed (farmers are not allowed to generate their own seed), teenagers raiding fields in the mistaken belief that marijuana is being grown, and great difficulties in exportation because of the necessity of convincing authorities that hemp is not a narcotic. Unless the producer participates in sharing of value-added income, large profits are unlikely. The industry widely recognizes that value added to the crop is the chief potential source of profit, as indeed for most other crops.

For a fiber crop, hemp is cut in the early flowering stage or while pollen is being shed, well before seeds are set. Tall European cultivars (greater than 2 m) have mostly been grown in Canada to date, and most of these are photoperiodically adapted to mature late in the season (often too late). Small crops have been harvested with sickle-bar mowers and hay swathers, but plugging of equipment is a constant problem. Hemp fibers tend to wrap around combine belts, bearings, indeed any moving part, and have resulted in large costs of combine repairs (estimated at $10.00/ha). Slower operation of conventional combines has been recommended (0.6–2 ha/hour). Large crops may require European specialized equipment, but experience in North America with crops grown mainly for fiber is limited.
